Fill out assessments online prior to appointments
Completing forms together can take up to an hour or more during an intake! The more you complete on your own time, the more it saves you.
Space appointments to every other week or monthly
We can discuss your needs and wants while completing the treatment plan.
Fill out application for sliding scale fees

Financial hardships may include but are not limited to low household income, college expenses, deployment, medical expenses, supporting an elder parent, etc. Please inform us of this of your intake paperwork.
